Forms 2016-17

The forms listed below are the ones you will need for fall 2016, spring 2017, and upon student eligibility for the summer 2017 semesters.

Step 1. Review the forms requested on your Financial Aid Communication Letter. If you do not have this letter you can determine which forms you need  by logging into Online Services. Once logged into Online Services click on the "Student" tab and locate the "Financial Aid missing documents" link under the Financial Aid section. On this screen you will will find all missing documents for your Financial Aid file.

Step 2. Complete Form 1 and all other forms listed in the above step.

Step 3. Promptly return all requested documentation to the Financial Aid Office. 

You will find a brief description below each form to help you understand why the form is being requested:

Financial Aid Information Form
 

All students seeking Financial Aid at Rock Valley College must complete the form listed below:

Form 1: Financial Aid Information Form - This is a six page document. Please print and turn in all six pages. This form explains in detail the academic standards all financial aid recipients must maintain in order to receive future aid as well as the applicant's rights and responsibilities, and other important information. Be sure to read the entire document and complete all areas of the form.

Verification Forms
 

Only complete the forms listed below if they are requested on your Communication letter you received in the mail or on your Online Services listed under "Financial Aid missing documents."

The forms below are required if your FAFSA form was selected for verification by the Department of Education, or if after review by the Financial Aid staff more information is required to resolve conflicting information within your file.

  • You are considered Independent if you were not required to provide parental information on the FAFSA.
  • You are considered Dependent if you were required to provide parental information on the FAFSA.
     

Form 2:Independent Standard Verification Form - Required for independent students in the standard verification group. Requires students to provide information such as household, work, and income information.

Form 3: Dependent Standard Verification Form - Required for dependent students in the standard verification group. Requires students and parents to provide information such as household, work, and income information.                     

Form 4: Independent Household Resources Verification Form - Required for independent students in the Household Resources Verification group. Requires students to provide information about the resources available to the household.

Form 5: Dependent Household Resources Verification Form - Required for dependent students in the Household Resources Verification group. Requires students to provide information about the resources available to their parents' household.

Form 6: Independent Child Support Paid Verification Form - Required for independent students in the child support paid verification group. Requires students to provide information about child support they may have paid.

Form 7: Dependent Child Support Paid Verification Form- Required for dependent students in the child support paid verification group. Requires students and parents to provide information about child support they may have paid.

Form 8:Independent Custom Verification Form-  Required for independent students in the custom verification group. Requires students to provide information such as; SNAP benefits, child support paid, and high school completion status.

Form 9: Dependent Custom Verification Form - Required for dependent students in the custom verification group. Requires students and parents to provide information such as SNAP benefits, child support paid, and high school completion status.

Form 10: Independent Aggregate Verification Form- Required for independent students in the aggregate verification group. This form requires students to provide information such as household, income, and SNAP information.

Form 11: Dependent Aggregate Verification Form - Required for dependent students in the aggregate verification group. Requires students and parents to provide information such as household, income, and SNAP information.

Form 12:Illinois Residency Verification List- Provides a list of documents approved by the state to verify Illinois residency for the Illinois state MAP grant.

Form 13: Curriculum Code Formonsists of a list of all programs that are eligible for financial aid. Form is required if you do not have an eligible code recorded in the RVC Records Office.

Form 14: Independent Low Income Verification Form - Students and parents may be asked to fill this form out if income reported appears to be unusually low. 

Form 15: Dependent Verification of Low Income Form - Student may be asked to fill this form out if income reported appears to be unusually low.

Form 16: Standards of Progress Appeal Form - Students receiving financial aid must make Satisfactory Academic Progress (SAP) toward the completion of course requirements in a certificate or degree program.  Fill out this form if you have been notified that you are no longer meeting the standards for financial aid and wish to appeal due to circumstances beyond your control.

Choose which method you would prefer for submitting this form:

Form 17:Permanent Standards of Progress Appeal - Students may receive Financial Aid until they have attempted 96 credit hours. This includes up to 43 transfer credits. Fill out this form if you have attempted more than 96 credit hours and wish to appeal this limit.

Form 18: 2016-2017 FAFSA Signature Form - Students and parents (if student is dependent) will receive this form if they did not sign the FAFSA electronically or did not submit a paper signature to the Department of Education.

Form 19: Enrollment History - Students are required to complete this form based on prior college enrollment.

Form 20: Dislocated Worker Verification - Students eligible for the Illinois Map Grant whom qualify under the Dislocated Worker status must complete this form.

The Department of Education mandates that colleges obtain specific documentation when verifying information that students submit when applying for financial aid. Rock Valley College also requires that students read their Rights and Responsibilities and the Standards of Academic Progress policy. Depending on your individual circumstance, additional information may be necessary to process and award your financial aid.
